We are looking to recruit a full time Team Leader for Supportlinks to lead a dynamic team of staff.
Supportlinks provides Needs Assessment and Service Coordination services to people with long term conditions, acute/short term needs, older people, mental health and addictions and provides dementia education.
The role includes the day to day operational management, preceptoring, supervision, modelling best practice and supporting staff in the community/inpatient setting. The role works collaboratively with other
coordinators and cluster leadership to work as integrated services.
The role is based in Palmerston North. Applicants will have previous experience or knowledge of interRAI, understanding of NASC systems and processes and hold an annual practicing certificate in any of the following disciplines, Nursing, Social Work, Occupational or Physiotherapy therapy. Prior experience working within Older Persons services, Mental Health and addiction or disability related fields.
