P2A Programme - Paediatric to Adult Transition
We are looking for a highly motivated registered nurses to work 0.5 FTE in a new and exciting programme that will support transition of young people in the Paediatric Services to the Adult Services across all specialities within MidCentral DHB and General Practice teams.
This will be a key role in engaging with adolescent patients to develop greater self-management skills and increased knowledge of their healthcare, along with the provision of general support and guidance during the transition process.
The RN P2A programme will work closely with all members of the Paediatric Service team to develop the service, support and empower young people on the transition pathway, provide ongoing education for staff, form strong relationships across the Adult Specialty teams and General Practice teams.
Looking for registered nurses preferably with
- Some experience in youth mentoring or leadership roles
- Knowledge of the interface between paediatric and adult services and General Practice teams and be able to work alongside colleagues in adult services to ensure best outcomes for the young person
- Professional portfolio at level 3
