If you are applying for a position as a Consultant, Specialist, Medical Officer of Special Scale (MOSS), Registrar, Senior House Officer or House Surgeon, then you will also be required to fill in the following forms: Declaration of any loss or surrender of medical license to practise or hospital suspensions and Release of Liability.
