Patient Information Guide

The MidCentral DHB Patient Information Guide includes details such as visiting and meal times, what to bring, communication and discharge information and more.
 
The guide will help to explain some of the hospital processes as well as what you can expect from services provided. You can also access a map of your Ward to help you ‘get about’ and also to assist those who might be visiting. 

My Health Passport
"My Health Passport​” is a booklet that has been created by the Health and Disability Commissioner (HDC) that you can carry with you when you visit health and disability services. The booklet​ has information about how you want people to communicate with you and support you when you use health or disability services.

The HDC has produced a HDC Going to Hospital​ booklet for the general public about what to expect when going to hospital. More information, including specific sections and a full copy of the booklet is available from the HDC site:

Most documents are available in Acrobat (PDF) format. Some documents are also available in other formats (doc, xls, html etc.) or as a printed version (hard copy). Please note, due to cost, a different format or printed version may not be available.
