This transparency statement explains how Te Whatu Ora Health NZ - MidCentral collects, uses and shares information.
   
The statement ensures information gathered about members of the public or other entities (directly or indirectly) is in accordance with the Information Gathering Model Standards issued by the State Services Commission.
 
MidCentral does not have any law enforcement responsibilities, and does not gather any information for these purposes.
 
The security of our premises is managed in accordance with the mandatory requirements outlined in the NZ Government’s Protective Security Requirements   and the government procurement standards.  Where we have serious concerns regarding potential threats to the physical security of staff, or the security of information or places, we will engage an appropriate authority (such as the NZ Police or Campus Security).
 
Our Role
MidCentral is responsible for providing or funding the provision of health services in the geographical area that stretches across the North Island of New Zealand from the west to the east coast and is distinguished by the Tararua and Ruahine ranges that traverse the centre of the district. MidCentral’s district comprises the following territorial local authority districts:
  • Horowhenua district
  • Manawatu district
  • Palmerston North city
  • Tararua district
  • The Otaki ward of the Kapiti coast district
In this role, MidCentral receives information on matters related to individual and public health gathered by other agencies. MidCentral uses this information to assess and provide health care services and coordinates with other agencies in preparedness and response to health concerns and events nationally.
 
How we collect this information
We collect information from individuals and agencies in accordance with our Health Information Collection guide and in accordance with the Health information Privacy Code (1994), the Health and Disability Commissioners Act (1994), Health and Disability Commissioner Regulations (1996) and the New Zealand Public Health and Disability Act 2000. 
 
How we secure this information
The security of our information is managed in accordance with our Information Technology Systems security and Access Policy and our Privacy Policy in compliance with the Privacy Act 1993, the Official Information Act 1982 and the Public Records Act 2005.
 
How we use this information
The information we hold is used only for the purpose for which it is collected or as otherwise advised, including as described in our privacy policy. These policies are regularly reviewed to ensure compliance with the law, our internal policies, guidelines and general risk management requirements.
 
Should we identify a need to gather information in scope of the model standards in future, we will ensure compliance with Information Gathering Model Standards, and update this transparency statement.
 
This statement applies to information gathered by us, our contractors, or any other third parties engaged by us.
