What are Integrated Family Health Centres?
An integrated family health centre (IFHC) is a new way of working to create a patient centred model of care with facilities provided to support the delivery and development of health services.
The model of care is about creating a better patient experience and proactively putting the patient first. It’s main aim is a common focus for health practitioners to simplify and enhance the experience of the patient and their journey through the health system, particularly between traditional primary (community) and secondary care (hospital) health services.
At MidCentral DHB the philosophy of “From population health to last days of life” reflects day-to-day practice and an integrated approach to health care. The philosophy embraces the four main types of integration:

Integration of General Practitioners (GPs) and other Primary Health Care professionals in the Primary Health Care team

Integration of the Primary Health Care team with other health care professionals working in the community (for example, district nurses, allied health professionals and clinical pharmacists)

Integration of the Primary Health Care team, other health care professionals working in the community and social care professionals

Integration of the Primary Health Care team, other health professional working in the community, social care professionals and hospital-based specialists.
Horowhenua Health Centre is the first health facility in the MidCentral DHB region to implement an integrated family health centre approach.
