Horowhenua Adult Community Mental Health Team is based at Horowhenua Health Centre.
The team also operates a outreach clinic out of the Otaki Women’s Health Centre. This service covers the Horowhenua District from Te Horo to Foxton, including the towns of Otaki, Levin, Shannon, Foxton and associated beach communities.
The service provides culturally appropriate assessments and treatment for adults from 18 years old to 65 years, suffering from a moderate to severe level of mental illness. The Horowhenua team is a multi disciplinary team that aims to provide a free quality recovery focussed service.
The team offers initial appointments with a mental health clinician to work with the person to decide what are the current issues, and what is the most appropriate support to enhance a recovery focussed plan.
This plan can include:
- Supportive care/treatment during acute phases of unwellness
- Medication prescribing and monitoring
- Individual therapy e.g. DBT Dialect Behaviour Therapy
- CBT Cognitive Behaviour Therapy
- Relapse prevention planning
- Recovery focussed work with individual clinicians
A referral can be from the person themselves, family/Whanau, General Practitioner (GP). It is generally preferred that referrals come from GP’s as it is considered important that they are aware and involved in the integrated treatment planning and care.
About the Team
The Horowhenua team consists of a clinical manager, consultant psychiatrist, medical officer special scale (Moss), psychologist, social worker, community psychiatric nurses, administrator/typist and community support worker.
