Service Coordination

Service Coordination supports consumers of the Mental Health and Addiction Service to gain independence in the community.

The service is managed by a designated service coordinator who supports clinicians, providers and clients to link with appropriate services as per consumer need.

All supports are recovery focussed with regular reviews by Service Coordination.



Supported Accommodation

Consumers that are unable to live independently in the Community can access supported accommodation through Service Coordination via the Community Mental Health Teams. Consumers are assessed at different levels as per the Support Needs Assessment Tool.
 
Level 2  Provided by Manawatu Accommodation and Sheltered Housing Trust (MASH)
Consumers are responsible for household chores and cooking in a flatting situation. A support worker visits daily to oversee any problems.

Level 3 + 4 Provided by MASH and St Dominics (Feilding)
MASH - Consumers are supervised with cooking and cleaning, staff are present all the time and night staff are sleepover staff only.
St Dominics – Consumers have meals provided and are only responsible for cleaning room and laundry. Staff are present 24 hours per day.

MASH has 24 hour on call registered nurses whereas St Dominics have onsite registered nurses during business hours with on call through the night.

Consumer’s Support Needs levels are reassessed annually by Service Coordination and clients are moved according to reassessment.

Recovery Packages

Recovery Packages are provided by MASH and access is through Service Coordination. Packages of Care are wrapped around consumers who may need some short term intensive supports. This is individually focussed to enable a client to stay in the community or assist in early discharge from the Acute Mental Health Ward.

Support in the Community Service

Support in the community is provided by MASH and is accessed through Service Coordination via keyworkers of the Community Mental Health Team. This service is also available to clients that have been discharged from Mental Health Services as appropriate with a support needs assessment from service coordinator.

Medication Management

Medication management is provided by MASH and access is through a keyworker and service coordinator who does the support needs assessment at 6 weeks using a step down process where support workers supervise the taking of medications reviewed by service coordinator.

Home support

Home support is provided by Healthcare NZ and accessed via keyworkers using the Support Needs Assessment Tool.

Respite

Respite care is provided by MASH, St Dominics and Te Matai and can be for a period of time out of up to one week. This service is accessed through keyworker to service coordinator. There are:

  • 2 beds at St Dominics and MASH for Adults
  • 5 beds at Te Matai for children 5-18 years of age


Carer Relief

Carer relief is available for CAFS clients only and is time out for parents who nominate a carer to look after child.

Access is through keyworkers and support needs assessment.
Entitlement of 14 days in 6 months. This is paid for by Service Coordination.
