Community Team
A multidisciplinary team of professionals from diverse disciplines who together provide comprehensive assessment and consultation to facilitate recovery where there is severe mental illness.
Services provided
Clinicians who specialise in provision of assessment & treatment for people with suspected or diagnosed severe mental illnesses, mental disorders/diagnoses, inclusive of co-existing and dual diagnosis.
Provision of Individual, group or family therapy, medication education and management, cognitive, behavioural and dynamic psychotherapy, education on mental health conditions.
Liaison for cultural assessment, specialist consultation, identification of strengths, support systems, co-ordination of treatment between several agencies and health providers crisis intervention, mental health rehabilitation, follow-up and outreach clinics, consultation and liaison services for other providers and community groups.

Partnership
- Assessment and treatment interventions are conducted in purposeful partnership with the service user, Family/Whanau/Significant Others

Mental Health Services encourage consumer participation in the development of a treatment plan (Integrated Treatment Plan). The purpose of the Integrated Treatment Plan is to promote and achieve recovery, and empower consumers to take responsibility for accomplishing good health. Indicators and expectations for working through periods of unwellness are also identified.

This is a free service and is offered 24 hours a day, seven days a week. Telephone 0800 653 357. During business hours, each community mental health team offers a mental health emergency service in their area. Existing consumers/tangata whaiora are encouraged to contact their key-worker in the first instance.
