MidCentral DHB General Enquiry/Feedback Form
Use this form to make a general enquiry or provide feedback for all MidCentral DHB services/departments. Your enquiry/feedback will be sent to the Communications Unit.
If your feedback is a compliment or a complaint about a health care service you have received please fill in this form.
Please Note:
- Provide as much information as you can to help us answer your enquiry. If you have a lot of text, please attach a file.
- Enquiries/feedback will be processed as soon as possible – usually the same day (Monday to Friday).
- This service is available between 8am to 5pm New Zealand time (+12 GMT or +13 GMT daylight savings) Monday to Friday. Enquiries sent in the weekend will be processed the following Monday.
- You should receive an acknowledgment confirming your enquiry/feedback has been received. If you don't receive an email, please check your junk email folder.
- If you require further assistance, please email the Communications Unit or refer to the Contacting Services/Departments page for a telephone number.
