How can I provide feedback, send a message to a patient, make a general enquiry or access my medical records?

MidCentral District Health Board provides a number of ways to communicate electonically via this website.

Send an email

If you wish to send an email, there will be a number of different emails available throughout the site, but if you're unsure which address to use, please send your email to communications@midcentraldhb.govt.nz

Fill in the appropriate form

A number of pages throughout the site have an electronic form available:

Apply for a job using the online vacancy form. Please view our current vacancies to apply.
Patient message form. Use this form to send a message to a patient who is at Palmerston North Hospital or Horowhenua Health Centre. Your message will be processed by central enquiries.
General feedback/enquiry form. Your request will be processed by the Communicaitons Unit.
Website feedback form. Your feedback will be processed by the site webmaster.
